Frantic attempts were made to resuscitate him. He was brought by ambulance to University Hospital Limerick where he was pronounced dead.
Jack’s parents, Kevin and Carmel Lyons and twin brothers Darragh and Cormac, aged 10 are being comforted by family and friends.
A postmortem is due to be carried out and a Health and Safety Authority investigation is underway.
