Medical Misadventure verdict in tragic Aoife’s death

The late Aoife Johnston.

AOIFE Johnston’s family have said that while they welcome the verdict of medical misadventure delivered by the Coroner today (Thursday) “it won’t bring Aoife back”.

The verdict came in after four days of hearing evidence  from medical staff and the Johnstons at the court in Kilmallock in the county .

University Hospital Limerick staff and former staff cried in the witness box as they described the events of the December 19 and 18 last, when the 16 year old Shannon schoolgirl lay dying of sepsis.

Her mother, Carol, said Aoife died “a horrible death,” and the verdict “doesn’t change anything,” for the family.

Coroner John McNamara on Thursday returned a verdict of medical misadventure and said there were “systemic failures, missed opportunities, breakdown in communication, clearly the bottom line is Aoife should have been seen and treated that is without doubt”.
